"K" of the 5th Regiment of New York Cavalry Volunteers, discharged with Surgeon's Certificate of Disability, 4th March 1862, Certificate and Form No. 5 included, signed by Howard, documents affixed at the upper left corner. Repairs have been made with modern archival tape for easier handling. I have carefully reinforced tears and folds for some of these with archival repair tape, for handling purposes.
